A very interesting graphic/whitepaper, with very interesting statistics regarding organizations that are effective with change management initiatives vs. organizations that are not very effective with change management initiatives.

The one missing essential ingredient that will ensure change management (individual behavior change), is coached, individual reinforcement, over time

Here is an article I penned specific to sales, but the concept applies for all behavior change,  detailing why change does not occur unless the reason for change is individually reinforced over time (note the article included on change of habits):
